The initial support only set guideTagOpen and guideTagClose for the html and xterm256 formats. The base generator skips guides when guideTagOpen is empty, so ansi, bbcode, latex, odt, pango, rtf, svg and tex drew none.

Set the guide tags in each generator, and emit the guide style next to the other style classes where the format defines them. The RTF colour table index is computed after the variable length keyword classes. ANSI has no theme driven RGB colour, so it falls back to a dim default foreground sequence.

Applies the all_formats.diff patch attached on !164.
